By Nobuko Kanaya The long-term care insurance system enacted in April 2000 introduced a quasi-market system in which a variety of public, non-profit, and for-profit providers compete with each other in order to efficiently supply high-quality long-term care services. This book analyzes and shows the behavior of non-profit and for-profit organizations from the introduction of the long-term care insurance service business in Japan more than 20 years ago until recently. What indicators can be used to compare and determine what is a good long-term care service for users? What are the differences and factors in the quality of services provided by for-profit corporations and non-profit corporations, which are difficult to treat as a single category, when viewed by market and management entity with regional differences? Also, is there cream-skimming oriented behavior occurring among providers? What factors affect the number of users? The report clarifies the relationship between multiple factors and service quality, and presents the advantages and disadvantages of increased efficiency and the outlook for the long-term care services market. The report also analyzes the significance of maintaining fairness* to users and not compromising the quality of services amidst the expansion of operators' profits, as well as the significance of protecting the working environment of nursing care workers. The research results should be used as a reference for the field to see how the system of competition between non-profit and for-profit organizations has functioned in the public service that is the long-term care insurance system. |
